In preparation for the chiropractic consultation, the patient will be asked to fill out forms with a series of background information about his/her symptoms & conditions. Each patient is encouraged to print and fill out forms at home prior to the initial visit. Patients will need to provide background history on any pre-existing medical conditions or prior injury, family history and previous and current healthcare providers and treatment.
After the initial intake forms are complete, the patient then has a consultation with the doctor. This is when the doctor and patient discuss medical history and major complaints. Based upon the patient’s history, diagnostic x-rays will be performed in the office to reveal pathologies and identifying structural abnormalities. This is when the office staff will go over first day costs and proceed after the patient agrees to continue.
The patient will undergo a thorough orthopedic/neurological exam performed by the doctor. After the exam, the doctor will then offer a light relief treatment for the day. All data gathered on the 1st visit will be assessed and a detailed treatment plan will be established and discussed in the second visit.
